There are 15.4 million people with a long-term condition in England. Numbers are expected to rise due to an ageing population and certain lifestyle choices that people make. NHS and social care services need to be prepared and responsive to meet the challenges that this may pose.
To support this, the Department of Health has developed a strategy for improving care for people with long term conditions, which is described in the Long Term Conditions Model.
